Job summary

Evolve Therapy Services is seeking a Physical Therapist for an in-house Skilled Nursing Facility partnership in Olivia, MN. This full-time role offers a $5,000 sign-on bonus plus tuition reimbursement and additional part-time or PRN options.

The incumbent will plan, organize, and direct PT services, ensuring compliance with current standards. Responsibilities include evaluation, treatment planning, documentation, and collaboration with an interdisciplinary team.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards.
• Adhere to procedures regarding documentation and billing of PT services.
• Demonstrate sound judgment in evaluation, planning, implementation and follow-up of therapy programs.
• Conduct screening of residents to determine need for intervention/treatment.
• Evaluate residents to obtain data for treatment planning.
• Develop treatment plans with short and long-term goals.
• Communicate findings and plan to residents, families, and team members.
• Complete compliant and medically necessary documentation per CMS expectations.
