Job summary

Evolve Therapy Services is hiring a Physical Therapist for an in-house Skilled Nursing Facility in Olivia, MN. The position is full-time, with additional Part Time or PRN roles available, and offers a $5,000 sign-on bonus plus tuition reimbursement.

The PT will plan, direct, and provide therapy services in accordance with regulatory standards and organizational procedures. The role requires a BS in Physical Therapy, an active license in the state, and ongoing continuing education.

Qualifications

• Minimum of Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.
• Commitment to compliant and medically necessary documentation.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Demonstrate sound judgment in evaluation, planning, implementation, and follow-up of resident therapy programs.
• Conduct screenings to determine intervention needs and evaluate data for treatment planning.
• Develop treatment plans with short and long-term goals and communicate findings to residents, families, and the interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR systems as required.
