Evolve Therapy Services is hiring a Physical Therapist for an in-house Skilled Nursing Facility in Olivia, MN. The role includes planning, implementing, and directing PT services to residents, while ensuring CMS-compliant documentation in NetHealth EMR.

Full-time is available with a $5,000 sign-on and tuition reimbursement; part-time and PRN positions are also offered. Licensure and ongoing professional development are required to provide high-quality resident care.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Maintain licensure and certification as required by state boards.

Responsibilities

• Provide physical therapy services to residents following established standards and procedures.
• Adhere to documentation and billing procedures for therapy services.
• Demonstrate sound clinical judgment in evaluation and plan development.
• Conduct screening of residents to determine need for intervention or treatment.
• Develop treatment plans with short- and long-term goals.
• Communicate evaluation findings and plans to residents, families, and the interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR as required.
