Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapist for an in-house Skilled Nursing Facility in Cedar Grove, NJ. The role centers on planning, directing, and delivering therapy services while adhering to federal, state, and local standards and CMS expectations.

You will work with an interdisciplinary team to optimize resident outcomes and ensure high-quality care. Qualifications include a BS in Physical Therapy, active state licensure, and ongoing CE.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents following standards and procedures.
• Document and bill therapy services in accordance with CMS expectations.
• Evaluate residents to determine treatment plans and goals.
• Develop short- and long-term treatment plans and communicate findings to families and the interdisciplinary team.
• Perform other duties as required to support resident care and organizational goals.
