Job summary

Evolve Therapy Services is hiring a Physical Therapist for an in-house Skilled Nursing Facility in Neptune, NJ. The role supports full-time, part-time, or PRN positions, providing therapy services to residents under CMS guidelines.

Responsibilities include evaluation, planning, implementation, and documentation of PT services, with emphasis on collaborative care and compliance with standard procedures and billing practices.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Provide physical therapy services to residents per established standards and procedures.
• Adhere to documentation and billing procedures for PT services.
• Demonstrate sound clinical judgment in evaluation, planning, and follow-up of therapy programs.
• Screen residents regularly to determine need for intervention or treatment.
• Develop treatment plans with short- and long-term goals and methods to achieve them.
• Interpret and communicate evaluation findings and treatment recommendations to residents, families, and interdisciplinary team.
• Complete compliant, medically necessary CMS documentation in NetHealth EMR.
