Job summary

Evolve Therapy Services is hiring a dedicated Physical Therapist for an in-house Skilled Nursing Facility in West Orange, NJ. You will evaluate residents, develop treatment plans, and document care in our CMS-compliant NetHealth EMR system.

Join a collaborative interdisciplinary team, provide high-quality resident care, and participate in ongoing professional development. Active PT license in NJ is required, with BS in Physical Therapy from an accredited program.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.
• CMS/NetsHealth documentation compliance.

Responsibilities

• Provide physical therapy services to residents according to standards and procedures.
• Adhere to documentation and billing procedures for therapy services.
• Evaluate residents to develop treatment plans with short and long-term goals.
• Develop and implement treatment plans and monitor progress.
• Communicate findings and plans to residents, families, and interdisciplinary team.
