Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Provide physical therapy services to residents following established standards and procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Evaluate residents to obtain data for treatment planning and implementation.
• Develop treatment plans with short- and long-term goals and methods to achieve them.
• Communicate evaluation findings and treatment recommendations to residents and interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR as required.
