Evolve Therapy Services is seeking a Physical Therapy Assistant for an in-house Skilled Nursing Facility located in Memphis, TN. The role supports a Physical Therapist and focuses on resident mobility, strength, balance, and functional goals.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Commitment to ongoing continuing education and licensure maintenance.

Responsibilities

• Provide physical therapy services under the supervision of a Physical Therapist.
• Conduct screenings to determine need for intervention and treatment.
• Develop and follow treatment plans with goals (short-term and long-term).
• Deliver individual and group sessions and assist with modalities and activities.
• Establish and modify resident schedules in coordination with the care team.
• Ensure timely, compliant documentation in CMS-aligned EMR systems.
