Job summary

Evolve Therapy Services is seeking a Physical Therapist for an in-house Skilled Nursing Facility partner in Memphis, TN. This full-time position (or PRN) focuses on planning, implementing, and directing physical therapy services to residents in accordance with federal, state, and local standards.

Responsibilities include evaluating residents, developing treatment plans with measurable goals, performing screenings, and ensuring accurate CMS documentation in NetHealth EMR.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Continued professional development to maintain licensure.

Responsibilities

• Provide physical therapy services per established standards and productivity targets.
• Adhere to documentation and billing procedures for therapy services.
• Evaluate residents and develop treatment plans with short- and long-term goals.
• Communicate evaluation findings and treatment plans to residents, families, and the interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR as required.
• Screen residents to determine intervention need and conduct follow-up evaluations as needed.
