Job summary

Evolve Therapy Services, a therapy management organization, is seeking a licensed Physical Therapist to join our in-house team at a Skilled Nursing Facility in Webster, MA. The role focuses on evaluating residents, developing treatment plans, and delivering care that aligns with CMS and professional standards.

You will document progress, participate in interdisciplinary rounds, and collaborate with families and care teams to ensure high-quality resident outcomes.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and licensure maintenance.

Responsibilities

• Provide physical therapy services to residents per established standards and procedures.
• Document and bill for services according to CMS expectations and internal policies.
• Develop treatment plans with short- and long-term goals and communicate findings to residents and the care team.
• Conduct screenings and evaluations to determine intervention needs and plan accordingly.
• Complete compliant documentation in NetHealth EMR systems.
