Evolve Therapy Services is seeking a compassionate Physical Therapy Assistant to support an in-house Skilled Nursing Facility in Grand Bay, AL. This role involves assisting a Physical Therapist, conducting screenings, and delivering both individual and group therapy focused on strength, mobility, balance, and functional goals.

The ideal candidate holds an Associate's degree in Physical Therapy, an active PTA license, and is committed to ongoing education to maintain licensure.

Formación

• Must hold an Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Continuing education to maintain licensure and certification.

Responsabilidades

• Provide physical therapy services to residents under supervision of the Physical Therapist.
• Conduct resident screenings to determine need for intervention/treatment.
• Assist with evaluation and treatment planning under PT direction.
• Create target goals and treatment plans with PT input.
• Deliver individual and group treatment covering strength, mobility, gait, ROM, balance, and modalities.
• Assist in selecting techniques and media and adapt as needed.
• Monitor resident responses and consult PT for modifications.
• Coordinate resident schedules with PT and team members.
• Adhere to schedules and adjust as needed for treatment regimens.
• Educate residents, families, and staff on equipment use.
• Participate in resident and staff education.
• Document compliantly in CMS expectations in NetHealth EMR.
