Evolve Therapy Services in Tampa, FL is seeking a compassionate Physical Therapist to join our in-house Skilled Nursing Facility team. This position supports residents with high-quality rehabilitation services and works with an interdisciplinary team to meet care goals.

The role emphasizes evaluation, treatment planning, and CMS-compliant documentation in NetHealth EMR, with options for full-time, part-time, or PRN schedules. A BS in Physical Therapy and active FL license are required.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure.
• Commitment to ongoing professional development and CMS guidelines.

Responsibilities

• Provide PT services to residents per standards, procedures, and productivity goals.
• Adhere to documentation and billing procedures for therapy services.
• Demonstrate sound judgment in evaluation, planning, and follow-up of therapy programs.
• Screen residents regularly to determine need for intervention/treatment.
• Screen residents referred for therapy to determine need for treatment.
• Evaluate residents to collect data for treatment planning and implementation.
• Conduct specialized evaluations as indicated.
• Develop treatment plans with short and long-term goals.
• Communicate findings and recommendations to residents, families, and the team.
• Perform other duties to support the organization’s goals.
• Complete compliant, medically necessary documentation per CMS and NetHealth EMR.
