Job summary

Evolve Therapy Services is seeking a Physical Therapist to provide in-house skilled nursing facility services. You will plan, organize, and direct physical therapy to residents, ensure adherence to CMS and company standards, and collaborate with an interdisciplinary team to optimize care.

The role requires a BS in Physical Therapy, active licensure, and ongoing continuing education. Experience with EMR documentation and CMS guidelines is preferred.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents per standards and procedures.
• Adhere to documentation and billing procedures.
• Demonstrate sound clinical judgment in evaluation and planning of therapy programs.
• Conduct regular screenings to determine intervention needs.
• Evaluate residents for treatment planning and implementation.
• Develop treatment plans with short and long-term goals.
• Communicate findings and plans to residents, families, and the team.
• Complete CMS-compliant documentation in NetHealth EMR.
