Job summary

Evolve Therapy Services is seeking a dedicated Physical Therapist for an in-house Skilled Nursing Facility in Queens Village, NY. The role supports full-time, part-time, and PRN schedules, providing comprehensive PT services to residents and coordinating with an interdisciplinary team.

Responsibilities include evaluation, plan development, documentation, and CMS-compliant record-keeping in NetHealth EMR. A BS in Physical Therapy and active license are required, with ongoing CEU commitments.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program
• Active Physical Therapy license in state(s) of practice
• Ongoing continuing education to maintain licensure and certification

Responsibilities

• Provide physical therapy services to residents in accordance with established standards.
• Adhere to documentation and billing procedures for PT services.
• Evaluate residents to develop treatment plans with short and long-term goals.
• Develop treatment plans and communicate findings to residents and interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR.
