Evolve Therapy Services is seeking a Physical Therapist for an in-house Skilled Nursing Facility in Olivia, MN. This full-time position offers a $5,000 sign-on bonus and tuition reimbursement, with additional Part Time or PRN opportunities.

The role focuses on planning, organizing, developing, and directing physical therapy services to ensure high-quality resident care while complying with CMS expectations and facility procedures. Strong assessment and communication skills are essential.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Evaluate residents to obtain data for treatment planning and implementation.
• Develop treatment plans with short and long-term goals and monitor progress.
• Communicate findings and recommendations to residents, families, and interdisciplinary team.
