Qualifications

• Active and valid Texas Physical Therapist license.
• Experience in SNF or long-term care preferred.
• Knowledge of physical therapy treatment practices and documentation.

Responsibilities

• Consult with patients to assess their physical condition and functional limitations
• Evaluate and interpret patient assessments and test results
• Develop individualized treatment plans using a variety of therapy techniques
• Create personalized rehabilitation programs tailored to each patient’s goals
• Administer medically prescribed physical therapy treatments
• Monitor patient response and modify approaches in collaboration with the care team
• Provide clinical education to patients, families, and caregivers
• Complete documentation in compliance with policies and regulations
• Attend required meetings with the Director of Rehab
