Ideal candidates hold a valid Texas PT license and have experience in SNF or long‑term care environments, with strong communication and collaboration skills to work with the interdisciplinary team.

Qualifications

• Active and valid Texas Physical Therapist license.
• Experience in Skilled Nursing Facility (SNF) or long‑term care preferred.
• Strong communication and collaboration skills.

Responsibilities

• Consult with patients to assess their physical condition and functional limitations.
• Evaluate and interpret patient assessments and test results.
• Develop and implement individualized treatment plans to improve mobility and function.
• Administer PT treatments per plan of care and monitor patient progress.
• Document thoroughly and in compliance with policies and regulations.
