Creative Solutions in Healthcare, Inc. is seeking a dedicated Physical Therapist (PT) to join our in-house rehab team in Texas. You will assess patients, design individualized treatment plans, and deliver therapy to improve mobility, function, and quality of life for residents in our care.

Qualificações

• Active Texas PT license
• SNF or long-term care experience preferred
• Knowledge of current PT practices
• Familiarity with Net Health EHR

Responsabilidades

• Consult with patients to assess physical condition and functional limitations.
• Evaluate and interpret assessments and test results.
• Develop individualized treatment plans using various therapy techniques.
• Create personalized rehabilitation programs tailored to patient goals.
• Administer prescribed physical therapy treatments to relieve pain and restore function.
• Ensure treatments follow the established plan of care and state/federal regulations.
• Monitor patient response and adjust approaches with the interdisciplinary team.
• Provide education to patients, families, and caregivers.
• Document thoroughly and timely according to company policies.
