Job summary

Creative Solutions in Healthcare, Inc. is seeking a dedicated Physical Therapist (PT) to join our in-house team. You will help residents improve mobility, strength, and overall quality of life through person-centered care.

Responsibilities include assessing patients, interpreting results, and developing individualized treatment plans. Must hold a Texas PT license and have SNF or long-term care experience preferred. Net Health EDD experience is a plus. Full-time or PRN positions available.

Qualifications

• Active and valid Texas Physical Therapist license.
• In good standing with regulatory agencies and licensing boards.
• Skilled Nursing Facility (SNF) or long-term care experience preferred.
• Current knowledge of physical therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.

Responsibilities

• Consult with patients to assess their physical condition and functional limitations.
• Evaluate and interpret patient assessments and test results.
• Develop individualized treatment plans using a variety of therapy techniques.
• Create personalized rehabilitation programs tailored to each patient’s goals.
• Administer medically prescribed physical therapy treatments to relieve pain, restore function, and promote healing.
• Monitor patient response to treatment interventions and modify approaches as clinically appropriate in collaboration with the interdisciplinary team.
• Complete documentation thoroughly and timely in compliance with company policies and state/federal regulations.
• Attend required meetings as designated by the Director of Rehab.
