Job summary

CareOne is seeking a Physical Therapist to provide in-house rehabilitation services at CareOne at Essex Park in Beverly, MA. The PT collaborates daily with nursing and clinical staff to achieve strong patient outcomes and maintain consistency as part of our team.

You will evaluate residents, develop and monitor treatment plans, and coordinate care with other disciplines while ensuring compliance with center and state standards.

Qualifications

• Master's or Doctorate Degree in Physical Therapy.
• Current and Active Physical Therapist license within the state of operations.
• Prefer experience/knowledge in older adult rehabilitation.
• Prefer knowledge of governmental and regulatory guidelines in designated healthcare settings.

Responsibilities

• Provides and directs resident rehabilitation care following accepted practice methods and standards.
• Evaluates residents and develops effective treatment plans.
• Coordinates supervision of treatment plans with all staff and disciplines.
• Screens residents according to directed schedules and practices.
• Delivers feedback to supervisor on clinical and rehab issues.
• Participates in center-led processes and services.
• Performs clinical documentation to meet requirements and maintains billing records.
• Participates in in-service training; may travel to other facilities for services.
