Job summary

CareOne Weymouth is seeking a Physical Therapist to join an in-house rehab team. The role focuses on providing high-level resident rehabilitation care within our in-house services, separate from outside contractors. Responsibilities include evaluating residents, creating and managing treatment plans, and coordinating with nursing and therapy staff.

Qualifications

• Licensed Physical Therapist in the state of operation.
• Master's or Doctorate Degree in Physical Therapy.
• Experience in older adult rehabilitation preferred.
• Familiarity with governmental and regulatory guidelines in healthcare settings preferred.

Responsibilities

• Provides and directs resident rehabilitation care within general practice methods, meeting standards.
• Evaluates residents and establishes effective treatment plans.
• Coordinates supervision of treatment plans with staff and other disciplines.
• Screens residents according to schedules and practices.
• Provides feedback to the supervisor on clinical and rehabilitation issues.
• Participates in center processes and services.
• Performs clinical documentation to meet regulatory requirements.
• Maintains billing records to meet regulatory requirements.
• Participates in in-service training and may travel to other facilities as needed.
