Evolve Therapy Services is seeking a dedicated Occupational Therapist to join their in-house skilled nursing facility team in Johnstown, PA. This PRN position focuses on planning and directing OT services, evaluating residents, developing treatment plans, and collaborating with an interdisciplinary team.

The role requires an active OT license in the practicing state, a degree in Occupational Therapy, and commitment to ongoing professional development.

Qualifications

• Degree in Occupational Therapy.
• Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education to maintain licensure and certification.

Responsabilités

• Screen residents to determine best intervention.
• Evaluate residents to develop treatment plans and goals.
• Interpret and communicate evaluation findings to residents, families, and the team.
• Create individual therapy schedules with the team.
• Document and bill OT services per plan.
• Perform duties to effectively treat patients.
• Supervise COTA and ensure compliant documentation and reimbursement.
