Evolve Therapy Services is seeking a compassionate Occupational Therapist for an in-house Skilled Nursing Facility in Bradford, PA. The role focuses on planning, organizing, and directing OT services to ensure high-quality resident care and adherence to regulations.

Qualifications

• Degree in Occupational Therapy.
• Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Conduct regular screenings of residents and determine interventions.
• Evaluate residents and develop treatment plans with goals.
• Communicate evaluation findings and plans to residents, families, and team.
• Establish resident therapy schedules with team members.
• Document and bill for occupational therapy services.
• Supervise COTA and ensure compliant documentation and reimbursement.
