Job summary

Evolve Therapy Services in Warren, PA is seeking a licensed Occupational Therapist to provide in-house therapy services at a Skilled Nursing Facility on a PRN basis.

You will screen residents, develop and implement treatment plans, communicate findings to residents and families, coordinate with the interdisciplinary team, supervise COTA as needed, and ensure proper documentation and billing.

Qualifications

• Degree in Occupational Therapy is required.
• Active Occupational Therapy license in practicing state(s) is required.
• Ongoing continuing education and professional development to maintain licensure and certification

Responsibilities

• Screen residents to determine the best intervention or treatment for each patient.
• Evaluate residents to inform and develop treatment plans and goals.
• Interpret and communicate evaluation findings and recommendations to residents, families, and the interdisciplinary team.
• Establish individual resident therapy schedules with other team members.
• Conduct therapy according to the treatment plan and ensure proper documentation and billing.
• Oversee COTA as needed and ensure documentation and treatments are carried out with clinical excellence.

Education
