Job summary

Evolve Therapy Services is seeking an Occupational Therapist for an in-house Skilled Nursing Facility in Johnstown, PA. This PRN (as-needed) role involves delivering OT services within regulatory guidelines and ensuring high-quality resident care.

The Clinician will screen, evaluate, and plan individualized therapies, coordinate schedules with the interdisciplinary team, and supervise COTA when needed to maintain clinical excellence and proper reimbursement practices.

Qualifications

• Must have an Active Occupational Therapy license in practicing state(s).
• Degree in Occupational Therapy is required.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Screen residents to determine best intervention or treatment.
• Evaluate residents to inform and develop treatment plans with goals.
• Interpret and communicate evaluation findings and treatment plans to residents, families, and team members.
• Establish individual therapy schedules with team collaboration.
• Conduct therapy and ensure proper documentation and billing of OT services.
• Perform all other duties to effectively treat patients.
• Oversee COTA and ensure documentation and treatments meet standards.
