Evolve Therapy Services in Johnstown, PA, is seeking a compassionate Occupational Therapist for an in-house Skilled Nursing Facility. This PRN role focuses on planning, directing OT services and ensuring quality resident care.

You will screen, evaluate, and develop treatment plans; collaborate with the interdisciplinary team; supervise COTA; document and bill services; and pursue ongoing professional development.

Qualifikationen

• Degree in Occupational Therapy.
• Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education to maintain licensure and certification

Aufgaben

• Conduct regular screenings of residents to determine appropriate interventions and treatments.
• Evaluate residents to inform and develop treatment plans with short- and long-term goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and the interdisciplinary team.
• Establish individual resident therapy schedules in cooperation with the care team.
• Conduct therapy according to the treatment plan and ensure proper documentation and billing of occupational therapy services.
• Perform all other duties as assigned to effectively treat patients.
• Oversee COTA as needed and ensure documentation and treatments meet reimbursement and clinical standards.
