Job summary

Evolve Therapy Services is seeking a dedicated Occupational Therapist to join our in-house Skilled Nursing Facility in North Platte, NE. This full-time role involves planning, directing, and delivering occupational therapy services to residents, with a focus on quality care and regulatory compliance.

The OT will screen residents, develop treatment plans, coordinate with an interdisciplinary team, and supervise COTA as needed.

Qualifications

• Must have an active Occupational Therapy license in the practicing state(s).
• Ongoing continuing education and professional development to maintain licensure and certification.
• Degree in Occupational Therapy is required.

Responsibilities

• Conduct regular screenings of residents to determine best intervention or treatment.
• Evaluate residents and develop treatment plans with short- and long-term goals.
• Interpret and communicate evaluation findings and plans to residents, families, and team members.
• Establish individual resident therapy schedules with other team members.
• Perform therapy according to treatment plan and ensure proper documentation and billing.
• Oversee COTA as needed and ensure documentation and treatments follow reimbursement practices.
