Job summary

Evolve Therapy Services, LLC, based in North Platte, NE, is seeking an Occupational Therapist for an in-house Skilled Nursing Facility. You will plan, direct, and deliver OT services to residents, with emphasis on quality care and compliant documentation.

Full-time opportunities include a $10k relocation bonus and flexible scheduling (PRN options). The role requires current OT licensure, ongoing professional development, and collaboration with a multidisciplinary team to improve resident

Qualifications

• Degree in Occupational Therapy
• Active OT license in practicing state(s)
• Ongoing continuing education and professional development to maintain licensure and certification

Responsibilities

• Screen residents to determine intervention needs.
• Evaluate residents and develop treatment plans with goals.
• Communicate findings, plans, and recommendations to residents, families, and team.
• Schedule resident therapy sessions with the team.
• Deliver therapy per plan and document/bill services.
• Perform other duties to effectively treat patients.
• Oversee COTA and ensure documentation and reimbursement practices are followed

Education

Degree in Occupational Therapy

Active Occupational Therapy license in practicing state(s)

Ongoing continuing education and professional development
