Job summary

Evolve Therapy Services is seeking a compassionate Occupational Therapist for an in-house Skilled Nursing Facility in Kenosha, WI. The position is PRN with flexible or casual hours available.

The Occupational Therapist will plan, organize, develop, and direct OT services ensuring compliance with federal, state, and local standards, and collaborating with the administrator to maintain high resident care quality.

Qualifications

• Degree in Occupational Therapy required.
• Active OT license in practicing state(s) required.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Conduct regular screening of residents to determine best intervention or treatment for each patient.
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members.
• Establish individual resident therapy schedules in cooperation with other team members.
• Conduct therapy according to treatment plan and ensure proper documentation and billing of occupational therapy services.
• Perform all other duties as assigned to effectively treat patients.
• Oversee COTA as needed and ensure documentation and treatments are carried out with clinical excellence and proper reimbursement practices.

Education

Degree in Occupational Therapy

Active Occupational Therapy license in practicing state(s)
