Job summary

Evolve Therapy Services is seeking an Occupational Therapist for an in-house Skilled Nursing Facility located in Wisconsin. Roles include planning, directing OT services for residents, and collaborating with administrators and the care team.

Full-time, part-time, or PRN positions are available; candidates must hold an active OT license in Wisconsin and participate in ongoing professional development. Competitive benefits are provided.

Qualifications

• Degree in Occupational Therapy is required.
• Active OT license in Wisconsin (or practicing state) is required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine best interventions or treatments.
• Evaluate residents to inform and develop treatment plans and goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents and families.
• Establish individual resident therapy schedules with the care team.
• Deliver therapy per plan and ensure proper documentation and billing of OT services.
• Oversee COTA as needed and ensure documentation and treatments meet reimbursement standards.

Education

Degree in Occupational Therapy

Active OT license (Wisconsin)
