Job summary

Evolve Therapy Services, LLC is seeking an Occupational Therapist for an in-house Skilled Nursing Facility in Kenosha, WI. This PRN role supports consistent or casual hours to meet resident needs.

You will plan, organize, and direct occupational therapy services, work with a multidisciplinary team, establish treatment plans, document care, and supervise COTA as needed to ensure high quality resident care and regulatory compliance.

Qualifications

• Active Occupational Therapy license in Wisconsin
• OT degree from an accredited program
• Commitment to ongoing continuing education

Responsibilities

• Plan, organize, develop, and direct Occupational Therapy services
• Screen residents and develop treatment plans with goals
• Communicate findings to residents, families, and team members
• Schedule therapies with other team members
• Document therapy sessions and bill appropriately
• Supervise COTA and ensure compliant care
