Job summary

Evolve Therapy Services seeks a compassionate Occupational Therapist to provide in-house therapy services at a Skilled Nursing Facility in Kenosha, WI. The PRN role offers flexible hours, with consistent or casual scheduling as needed to support resident care.

You will plan, implement, and supervise treatment plans, collaborate with the interdisciplinary team, document progress, and ensure compliant billing. Active WI OT license and SNF experience preferred.

Qualifications

• Active Occupational Therapy license in practicing state(s).
• Degree in Occupational Therapy.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine best intervention or treatment.
• Evaluate residents and develop treatment plans with goals.
• Communicate findings and plans to residents, families, and team members.
• Establish resident therapy schedules with team members.
• Conduct therapy per plan and ensure proper documentation and billing.
• Perform other duties to effectively treat patients.
• Oversee COTA and ensure documentation and treatments meet reimbursement practices.
