Job summary

Evolve Therapy Services, LLC in Kenosha, WI, is seeking an Occupational Therapist to provide in-house SNF services on a PRN basis. You will plan, direct, and deliver therapy to residents in accordance with standards and regulations, ensuring high-quality resident care.

The role requires an OT degree and an active license in Wisconsin, with ongoing continuing education. You will screen, evaluate, and develop goals, coordinate with the team, document appropriately, and supervise COTA as needed.

Qualifications

• Degree in Occupational Therapy
• Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain licensure and certification

Responsibilities

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed
