Job summary

Evolve Therapy Services, LLC is seeking an Occupational Therapist for an in-house skilled nursing facility in Kenosha, WI. This PRN position offers consistent or casual hours and requires a licensed OT to deliver high-quality resident care.

You will conduct screenings, develop and implement treatment plans, and coordinate with an interdisciplinary team. Ongoing licensure and professional development are expected, with supervision of COTAs as needed.

Qualifications

• Bachelor of Occupational Therapy or equivalent degree.
• Active OT license in the state(s) where practicing is required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine the best intervention or treatment.
• Evaluate residents to inform and develop treatment plans with goals.
• Communicate evaluation findings and recommendations to residents, families, and team members.
• Establish individual therapy schedules with other team members.
• Document and bill occupational therapy services per plan.
• Perform duties to effectively treat patients.
• Oversee COTA and ensure proper documentation and reimbursement practices.
