National Youth Advocate Program is seeking a full-time Family Services Coordinator in Eaton, OH to advocate for youth and families, provide in-home support, transportation, parenting education, counseling, and resource referrals. The role emphasizes community collaboration, safety, and adherence to the case plan.

The position requires a 4-year degree in a social services field, along with strong computer skills and willingness to work non-traditional hours to serve families in Preble County.

Qualifications

• Bachelor's degree in Social Work or comparable Human Services field from an accredited institution.
• 2 years of work experience in direct service with youth and families preferred.
• Willingness to work flexible/non-traditional hours.
• Ability to work with diverse cultures and backgrounds.
• Proficient use of computers, smartphones, and standard office software.

Responsibilities

• Actively supports and represents the organization's mission and values.
• Provides in-home supportive services, transportation, parenting education, counseling, and advocacy.
• Travels daily to deliver community-based services in compliance with requirements.
• Completes documentation and trainings in a timely manner.
• Coordinates services across mental health, medical, educational, and social domains.
• Reviews and updates youth-based plans to ensure progress.
• Maintains strong relationships with foster parents and families.
• Supports family reunification and stabilization per the case plan.
• Provides safety assessments, crisis response, and stabilization services as needed.
• Meets productivity thresholds and participates in CQI activities.
