Job summary

National Youth Advocate Program in Columbus, OH is seeking an Experienced Family Case Manager to support children and families with advocacy, services, and community resources. You will collaborate with youth, families of origin, foster parents, and partners to help ensure safe, stable living in the community.

The role requires a 4-year degree in social services, a willingness to work flexible hours, and strong computer skills.

Qualifications

  • Bachelor’s Degree in Social Work or comparable Human Services field from an accredited institution.
  • 2 years of work experience in direct service with youth and families preferred.
  • Willingness to work flexible and non-traditional hours in the service of foster caregivers, families and persons served.
  • Capacity to be flexible and responsive to youth served and diverse backgrounds.
  • Proficient use of desktop and laptop computers and common office software.

Responsibilities

  • Complete or revise the Family Risk Assessment with all family members living in the home to assess strengths and needs, risk of harm to the child/children, and monitor child safety.
  • Maintain contact with families through regular, planned, and unannounced visitations.
  • Provide case management, transportation, parent education, counseling, community resource linkage, advocacy, and other professional services.
  • Participate in administrative and court reviews of the case plan and other court proceedings.
  • Work with Foster Care Programs to coordinate visits with the Foster Treatment Coordinator.
  • Pursue permanent custody and planning for adoption or other permanent substitute care for children who cannot be returned to their birth families.
  • Perform duties on-call as outlined in the on-call description when called upon to respond to after-hours emergencies.
  • Travel daily to provide community-based services to, and on behalf of, youth and families in compliance with organizational, contract, and regulatory requirements.
  • Utilize Children Welfare Best Practice Principles in decision-making.
  • Participate in Continuous Quality Improvement activities on a monthly/quarterly basis.
  • Actively participate in the weekly supervision process.
