Phoenix Home Care & Hospice is seeking a motivated RN or LPN to join our in-home care team in Missouri. This part-time role offers meaningful, one-on-one patient visits in the comfort of clients' homes, managing vitals, medications, and labs while supporting independence and comfort.

You will contribute to high-quality, compassionate care and collaborate with a supportive in-home team. Flexible scheduling and comprehensive benefits accompany this opportunity.

Competenze

• Active Registered Nurse or Licensed Practical Nurse license in MO.
• Home care in-home experience preferred but not required.
• Strong organizational and customer service skills.

Mansioni

• Complete admission assessments and reassessments for in-home clients.
• Provide nurse visits for adults and seniors: vitals, medication management, diabetic care, labs.
• May supervise caregiver visits.
• Educate patients and families on medications.

Responsibilities:

• Complete admission assessments and reassessments for In-Home caregiver population.
• Provide nurse visits for adult and senior population: Taking vitals, medication management, monthly diabetic nails, and labs.
• May provide supervisory visits for caregiver services
• Provide instruction/ education on medication
• Good time management, detail oriented and self-starter

Requirements:

• Active Registered Nurse or Licensed Practical Nurse license in the state of MO
• Home Care experience preferred but not required
• Strong organizational and self-management skills
• Strong and compassionate customer service skills
• Valid driver’s license
