Job summary

Phoenix Home Care & Hospice in Ava, MO seeks an in-home registered nurse (RN) or licensed practical nurse (LPN) for part-time visits in Ava, MO and surrounding areas. You will assess, monitor, and support clients in their homes, delivering compassionate care and coordinating with caregivers.

Responsibilities include admissions and reassessments, vitals and meds management, diabetic nail care, and possible supervisory visits. Valid MO license and reliable vehicle required.

Qualifications

• Active RN or LPN license in Missouri.
• Home care experience preferred but not required.
• Strong organizational and self-management skills.
• Strong and compassionate customer service skills.

Responsibilities

• Complete admission assessments and reassessments for In-Home caregiver population.
• Provide nurse visits for adult and senior population: Taking vitals, medication management, monthly diabetic nails, and labs.
• May provide supervisory visits for caregiver services
• Provide instruction/ education on medication
• Good time management, detail oriented and self-starter
