Phoenix Home Care & Hospice is seeking an in-home Registered Nurse or Licensed Practical Nurse to join our caregiving team in Bolivar, MO and surrounding areas. As an RN or LPN, you will perform admission assessments, nurse visits, medication management, and caregiver supervision while delivering high‑quality care in clients’ homes.

Qualifications

• Active RN or LPN license in MO.
• Home care experience preferred but not required.
• Strong organizational and self-management skills.
• Strong and compassionate customer service skills.
• Valid driver’s license.

Responsabilités

• Perform admission assessments and reassessments for the in-home caregiver population.
• Provide nurse visits for adult and senior populations: taking vitals, medication management, monthly diabetic nails, and labs.
• May provide supervisory visits for caregiver services.
• Provide instruction/education on medication.
• Good time management, detail oriented and self-starter.
