Responsibilities

• Coordinate with healthcare, education, and employment services for participants.
• Identify transportation needs and preferences for participants.
• Provide safety education and personal safety training.
• Assist with daily living activities and self-care with supervision as needed.
• Develop personalized care plans and monitor progress toward goals.
• Support tenancy and advocate for community-based resources.
• Collaborate with client’s support network for consistency.
