A PLACE CALLED HOME is seeking a Direct Support Professional (DSP) to provide Individualized Home Supports (IHS) with Training in Scott County, MN. The role focuses on teaching clients to build daily independence in homes and communities, requiring dependable, patient, and motivated staff.

Responsibilities include teaching living skills, documenting progress, and following CSSP plans, with flexible scheduling and up to 40 hours per week.

Formación

• Experience working with individuals with disabilities preferred.
• Strong communication and documentation skills.
• Reliable transportation required.
• Must pass background study.
• Ability to work independently and follow care plans.

Responsabilidades

• Provide IHS with Training services in clients’ homes and community.
• Support and teach independent living skills (cooking, cleaning, budgeting, community integration, etc.).
• Follow each client’s Coordinated Service and Support Plan (CSSP).
• Document daily progress notes tied to client goals.
• Use EVV (Electronic Visit Verification) to track time and services.
• Maintain professional boundaries and ensure client safety at all times.
