Job summary

Creative Living Options seeks a Direct Support Professional – HSW to provide individualized Supported Living Services to adults with intellectual and/or developmental disabilities. You will implement support plans, assist with daily living tasks, mobility, and community participation while maintaining safety and confidentiality.

The role requires professionalism, sound judgment, and commitment to person-centered care in home and community settings.

Qualifications

• High school diploma or equivalent required.
• Experience supporting individuals with intellectual and/or developmental disabilities preferred.
• Ability to learn and implement specialized client-specific procedures and training requirements.
• Ability to adapt to varying levels of support needs and changing environments.
• Ability to remain calm and exercise sound judgment during emergencies.

Responsibilities

• Provide direct support and supervision in home and community settings.
• Assist with activities of daily living and mobility as trained.
• Support client independence and participation in meaningful activities.
• Maintain documentation and report changes to supervisors.
• Communicate effectively with the interdisciplinary team and family as appropriate.
