Job summary

CenterWell Home Health, a Humana company, seeks a Speech Language Pathologist to evaluate, direct and provide therapy to patients in home or facility settings. You will participate in care plans, perform assessments of speech, voice, language, hearing and swallowing, and coordinate with the supervisor and physician to promote rehabilitation and independence.

Requirements include ASHA CCC-SLP, at least six months of experience, a valid license and CPR, plus a valid driver’s license and auto

Qualifications

• ASHA CCC-SLP certification required.
• Minimum six months experience as a speech-language pathologist/therapist.
• Current and unrestricted licensure in state of practice.
• Current CPR certification.
• Valid driver’s license, auto insurance and reliable transportation.

Responsibilities

• Evaluate and provide speech-language pathology services to patients in home or facility settings.
• Develop and periodically review Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing to support rehabilitation.
• Administer and interpret diagnostic tests and apply therapeutic treatments including audiology screening.
• Document changes in patient condition and response to treatment with supervisor/physician.
