Job summary

Humana’s CenterWell Home Health is seeking a Speech Language Pathologist to deliver in-home and facility-based services. You will evaluate, diagnose and treat speech, language, voice and swallowing disorders, develop Plans of Treatment and Care, and support patient independence and quality of life.

The role requires ASHA CCC, a current license, CPR, and reliable transportation to visit patients. It’s a patient-facing, care-coordination position with strong emphasis on documentation and teamwork.

Qualifications

• Meet the education and experience requirements for Certification of Clinical Competence in Speech Language Pathology or Audiology granted by ASHA.
• Minimum six months experience as a speech therapist / speech language pathologist.
• Current and unrestricted license.
• Current CPR certification.
• A valid driver’s license, auto insurance, and reliable transportation are required.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing; administer and interpret diagnostic tests.
• Observe, record and report changes in the patient’s condition and response to treatment to supervisor and/or physician.
• Provide instruction and training to patients in use of alternative communication systems when appropriate.
• Maintain and submit documentation including reports, care plans and progress notes.
