Job summary

CenterWell Home Health is seeking a licensed Speech-Language Pathologist to provide in-home or facility-based care. You will evaluate and treat speech, language, voice, hearing and swallowing disorders, develop treatment plans, and document progress for patients managed at home.

The role emphasizes care coordination, patient education, and adherence to physician plans of treatment, within Humana’s CenterWell network of services.

Qualifications

• Minimum six months experience as a speech-language pathologist.
• Current and unrestricted license.
• Current CPR certification.
• Valid driver's license, auto insurance, and reliable transportation.

Responsibilities

• Evaluate, direct and provide speech-language pathology service to patients in the home or facility.
• Develop and review the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing to prevent, identify and minimize effects.
• Observe, record and report changes in patient's condition to supervisor and/or physician.
• Provide instruction and training to patients in use of alternative communication systems.
