Job summary

Humana's CenterWell Home Health seeks a Speech Language Pathologist to assess, diagnose and treat speech, language, swallowing, and related disorders in patients at home. You will develop and review the Plan of Treatment and coordinate with caregivers and physicians to optimize outcomes.

This patient-facing role requires ASHA CCC-SLP (or equivalent), at least one year in home health, an active license, CPR and a valid driver's license with reliable transportation.

Qualifications

Must have ASHA CCC-SLP certification or equivalent.
Minimum 1 year experience as a speech-language pathologist.
Current and unrestricted SLP license.
Current CPR certification.
Valid driver’s license with reliable transportation.

Responsibilities

Evaluate, diagnose and treat speech, language, cognitive, voice and swallowing disorders in home or facility.
Develop and review Plan of Treatment and Plan of Care.
Use professional judgment in assessment and treatment and document progress.
Administer and interpret diagnostic tests and therapies including audio-logic screening.
Maintain records and communicate changes to supervisor/physician.
Provide training to patients and families on communication strategies.
Coordinate care and participate in discharge planning.
