Job summary

BAYADA Home Health Care is seeking an experienced Speech Language Pathologist (SLP) for a per diem or part-time role providing home health visits to adults and geriatric patients in Northumberland and Cumberland counties.

Ideal candidates hold a PA license as an SLP and an ASHA CCC-SLP, with the ability to complete required credentialing and work within a multidisciplinary team. Home health experience is a plus, and EMR fluency is preferred.

Qualifications

• Current Pennsylvania licensure as Speech-Language Pathologist.
• Certification as SPEECH-Language Pathologist (CCC-SLP) or equivalent.
• Complete certification/exam process for SLP credentials.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated territories.
• Evaluate and diagnose speech, language, swallowing disorders; provide therapy.
• Develop and monitor individualized treatment plans.
• Revise care plans in collaboration with BAYADA clinicians.
• Educate clients and families on rehabilitative care and home safety.
• Document observations and interventions using a clinical EMR system.
