BAYADA Home Health Care is seeking an experienced Speech Language Pathologist (SLP) for per diem or part-time home health visits in Northumberland and Cumberland Counties, PA. The role involves evaluating and treating adults and geriatric patients with speech, language, or swallowing difficulties, building treatment plans, and collaborating with a multi-disciplinary care team.

Responsibilities include educating patients and families, ensuring safe home care, and documenting care in an EMR

Qualifications

• Current PA license as a Speech Language Pathologist.
• ASHA CCC certification or eligible for certification.
• Equivalent education/work experience required for CCC eligibility.
• Solid computer skills and EMR experience preferred.

Responsabilités

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations to diagnose and provide therapy for speech, language, swallowing difficulties.
• Develop a treatment plan based on evaluation, orders, and goals.
• Continuously assess and revise the care plan with the multidisciplinary team.
• Educate patients and families in rehabilitative care and safety.
• Document observations and interventions in a state-of-the-art EMR on a touchpad tablet.
