Zusammenfassung

CenterWell Home Health is seeking a licensed Speech-Language Pathologist to evaluate and provide speech/language pathology services in patients’ homes or facilities. You will participate in treatment planning, assess and treat communication and swallowing disorders, and document progress according to Humana’s policy.

The role requires ASHA certification eligibility, at least six months of SLP experience, current licensure, CPR, and a valid driver’s license with reliable transportation.

Qualifikationen

• Must meet the education and experience requirements for Certification of Clinical Competence in Speech Language Pathology or Audiology (ASHA).
• Minimum six months experience as a speech therapist/slp.
• Current and unrestricted license.
• Current CPR certification.
• A valid driver’s license, auto insurance, and reliable transportation are required.

Aufgaben

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and applications of therapeutic treatments including audiologic screening.
• Observe, record and report changes in the patient’s condition to supervisor/physician.
• Provide instruction and training to patients in alternative communication systems when appropriate.
• Provide counsel and instruction to patients, families and healthcare staff.
• Maintain and submit documentation per company policy.
• Prepare and submit written reports of evaluations, visits, care plans, and progress reports.
• Participate in care coordination and discharge planning.
