Job summary

CenterWell Home Health is seeking a Speech Language Pathologist to deliver in-home therapy, evaluating and treating speech, language, voice and swallowing disorders. You will participate in care plans, coordinate with physicians, and document progress and outcomes to support patient independence and recovery.

The role requires a valid license, CPR certification, and reliable transportation for home visits. Prior home health experience is preferred, with pay per visit and a field-based schedule

Qualifications

• Minimum six months experience as a speech therapist / speech language pathologist.
• Current and unrestricted license.
• Current CPR certification.
• A valid driver’s license, auto insurance, and reliable transportation are required.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing to prevent and minimize effects.
• Administer and interpret diagnostic tests and therapeutic procedures including audio logic screening.
• Observe, record and report changes in patient condition to supervisor/physician.
• Provide instruction in alternative communication systems when appropriate.
